We are happy to have you as a new Parishioner of our community. To help us serve you better, we ask that you fill out the registration form as completely as possible.

We wish to better serve your needs, so we ask that all parishioners register with the parish.  This can be accomplished by filling out a parish registration form and submitting it to the office. 

Forms can be picked up in the back of the church, from the parish office, or printed out from this website (see below).  After filling out the form, we kindly ask that you submit it to the parish office in one of several ways:

1.    Bring it to the parish office.  During business hours (9:00 a.m. -12:00 p.m. Monday through Friday), turn it in to a member of the parish staff.  Outside of business hours, deliver it via the mail slot in the parish office door.

2.    Bring the completed form to mass and put it into the collection basket.

3.    Mail the completed form to the parish office.

4.    Fax the completed form to the parish fax. (978) 365-5705
